How to set up your Twin Cheeks Cushion

“The last cushion you’ll ever have to buy.”

The Twin Cheeks Patented Pelvic Pain Cushion features two independent foam panels support the fleshy part of your buttocks.

Your sit bones rest just inside the open channel between them.

Your tailbone, perineum, and pudendal nerve pathway sit suspended in that channel — with the pressure redirected away from the areas that hurt.

It's a patented design. And it's the reason people who've tried every cushion on the market keep coming back to this one.

Raving Customer Reviews

  • "It comes with handles, and is easy to take on the go and for long car rides. It provides 100% pressure relief throughout the midline of your perineum and is good for both females and males. My patients with tailbone pain love it as well."

    — Rebecca Marcus, DPT, Core Dynamics Physical Therapy

  • "After buying many different kinds, the one I like the best is called the Twin Cheeks cushion."

    — HealthUnlocked Pelvic Pain Support Network User

  • "The typical donut cushion does not provide the appropriate pressure relief most patients with pelvic pain need. My personal favorite, and most popular cushion is the Twin Cheeks folding cushion. My patients with tailbone pain love it as well."

    — Hands and Heart Physical Therapist

  • "I've tried a variety of cushions that have failed at helping me sit but I've recently purchased the Twin Cheeks cushion and it's been AMAZING. I got the extra firm model and it's allowed me to sit for long periods of time without increasing my pain at all."

    — HealthUnlocked Pelvic Pain Support Network User

  • "After two years of pelvic pain, this is the first thing that's made sitting tolerable. My PT recommended it and she was right."

    — Michael R.

  • "I have 2 cushions and would not be able to sit at all without using one. I have the Twin Cheeks cushions. They are made to order and a bit expensive but for me they are worth every penny. I have been using these for about 18 months and plan to stock up if they decide to go out of business."

    — Pudendal Hope Forum User

  • "The Twin Cheeks has been my life saver. It's the extra firm low profile version. It folds down neatly and can be carried easily in a backpack. The Twin Cheeks cushion has allowed me to hang out (eat, movie theater) with my wife and family. It has also allowed me to keep flying when my job requires it."

    — Pudendal Hope Forum

  • "I use the Twin Cheeks Firm cushion to drive. It is adjustable and keeps the rectal area from touching the seat or bottom of the cushion."

Frequently Asked Questions

Everything you need to know before you order.

Woman sitting comfortably at desk using Twin Cheeks pelvic pain cushion
  • We make two. The Twin Cheeks is our flagship — two adjustable panels with an open channel. It works for the vast majority of conditions, and is what we recommend for almost everyone. The Simple Soother is a more affordable single-cushion option for central-only discomfort with no tailbone involvement. Still not sure? Just ask.

  • The panels need to be firm enough to hold your weight — if they compress too much, the channel closes and you lose the benefit. Here's a general guide:

    • Soft — under 120 lbs

    • Medium — 120 lbs to 150 lbs

    • Medium Plus — 130 lbs to 180 lbs (supportive base with a softer top layer)

    • Firm — 170 lbs to 200 lbs

    • Extra Firm — 200 lbs and up

    • Extra Cush 3" — over 150 lbs, extra support with a layer of extra soft foam on top

    • Low Profile — available in Medium Plus, Firm, and Extra Firm in a shorter profile, great for cars

Your cushion may feel quite firm at first but will soften and conform to your body within a couple weeks of use. Not sure? Just ask. We help people choose every day and are happy to exchange if needed.

  • Same adjustable dual-panel design, different height. The Standard is taller and works well for desk chairs and everyday use. The Low Profile sits only 1.5" off the seat, folds smaller, and is better suited for car seats or anyone who doesn't want to sit higher. Now available in Medium Plus and Firm in addition to Extra Firm.

  • Returns are accepted within 30 days of receiving your cushion. The cushion must be clean and in returnable condition. Return shipping labels are available for $8. We don't cover return shipping in order to keep our prices as low as possible for everyone.

  • Most cushions ship within 2–3 business days via UPS or USPS. Delivery times vary depending on your location. International orders ship via Global Post Standard Shipping — please be aware that customs fees may apply depending on your country.
